Please complete this form to the best of your ability and submit it to the City of Mukilteo. Staff reviews each request or complaint to determine whether it can be investigated by the City or referred to another agency. The goal of the City is to work with the property owner to obtain voluntary compliance. If a resolution cannot be reached, the City may begin formal enforcement proceedings.
Please select a topic from the drop-down list. If this is an emergency, please call 911. If this is a streets, sidewalks or stormwater utility issue, please complete the "Fix It Public Works!" form.
Please note that the City is unable to enforce disputes between neighbors, including but not limited to property line disputes, private easements, covenants or homeowner's association rules, hazardous trees on privately-owned adjacent property, or tenant-landlord disputes.
Please describe the issue you'd like us to know about. Include the address and owner or occupant's name if this request is about another property.
You may upload files or photos for staff to consider as they process your request.
All documents sent to the City of Mukilteo are public records and are subject to disclosure under the Public Records Act (RCW 42.56). The City may be required to release your name and/or information pursuant to RCW 42.56 or a court order. Pursuant to RCW 42.56.240(2), you, as the complainant, victim, or witness, may request nondisclosure of your identifying information.
Please provide a phone or cell phone number in case we have questions or want to notify you that your request has been completed.
Please note: Under Washington state law, email addresses are public records. If you do not want your email address released in response to a Public Records Disclosure Request, do not send electronic correspondence to the City. Instead, contact us by phone or in writing.
Note: If you do not provide an email address, we may not respond to your request. If this is a question about parking tickets, please contact the Police Department directly at 425-263-8100.
