Civil Rights and Title VI Complaint Procedures

Individuals or individuals as members of a specific class of persons, who feel they have been discriminated against with respect to city services or benefits on the basis of race, color, national origin, or sex may file a complaint with the City of Mukilteo.  Please consult our complaint procedure below.

Please refer to the City’s  Request for Accommodation should you seek access to services and programs for persons with a qualifying disability or would like the City’s complaint procedures in a different format.

Submission of Complaints

Complaints alleging discrimination may be submitted online or using a paper complaint form that should be drop off or mailed to City of Mukilteo, Title VI Coordinator, 11930 Cyrus Way, Mukilteo, WA  98275.  Both formats must be signed by the complainant or legal representative with contact information. You may also submit the form via email to titlevi@mukilteowa.gov using the subject of “Civil Rights Violation” or “Title VI Complaint”.

The complaint must contain information about the alleged discrimination such as the basis of the complaint, location, date and time of the incident, and the name of the alleged discriminating official if known and any witnesses to the occurrence.

The complaint should be submitted by the complainant and/or legal representative as soon as possible but no later than one hundred eighty (180) calendar days after the alleged violation to:

The City of Mukilteo will acknowledge receipt of the complaint within 10 business days, and inform the complainant of action taken or proposed action to process the allegation.

Investigation of Complaints

The City of Mukilteo will review complaints and may ask the complainant to provide additional information or for clarification if needed.  Within 60 days of receipt of the complaint, the Title VI Coordinator will conduct an investigation of the allegation and based on the information obtained, will render a recommendation for action as necessary.

Disposition of Complaints

Any written determination as to the validity of the complaint and a description of the resolution, if any, shall be issued by the Title VI Coordinator and a copy forwarded to the complainant no later than 90 calendar days after its filing.  Upon request, the determination will be made available in a format accessible to the complainant, such as large print, audio tape, sign language, or email. Appealing Disposition of Complaints

Complainants that are not satisfied with the disposition of the complaint by the Title VI Coordinator may appeal to the City Administrator or designee. The appeal should be made with the Title VI Coordinator within 10 business days of the date of the determination.  The request will be forwarded to the City Administrator or designee for a final determination.

The Title VI Coordinator shall maintain a record of each complaint and appeal, the City Administrator or designee’s response(s), and steps taken to resolve the complaint. The individual’s right to a prompt and equitable resolution of a complaint will not be impaired by his/her pursuit of other remedies. The use of this grievance process is not a prerequisite to the pursuit to other remedies.
